Summary:
The Transplant Data Coordinator is responsible for all data management activities for the transplant program.
The data manager will collaborate with the clinical team members to collect and verify data used for quality assurance and performance improvement monitoring.
The Data coordinator will collect, enter and verify clinical, administrative and demographic data using automated and manual systems, maintain system integrity and generate internal and external reports, as appropriate to maintain compliance with the CMS, TJC, DOH & OPTN The data coordinator troubleshoots IT needs for administrative and clinical team members and escalates to IT if unable to resolve.
Position ResponsibilitiesCompetently performs the duties related to management of the UNET and Renal Network and Transplant Database.
Abstracts data from medical records or other sources and enters in UNOS and Renal network database.
Maintains, updates demographic information in Renal Network system.
Generates reports of information to meet internal analysis or external regulatory requirements for DOH, CMS, OPTN & CMS.
Maintain database integrity by following appropriate procedures for system backup, editing and updating Ensures completion of all UNOS/OPTN forms through the UNET system, including but not limited to:
Transplant candidate registration, Transplant recipient registration, Transplant recipient follow up and living related donor forms Keeps current on changes/advances in the UNET system.
Collaborates with Transplant Program associates and others to ensure efficient, timely and accurate data entry and reporting.
8Updates and generates patient data on call report for Transplant Coordinators Copies and prepares medical records for insurance agencies, regulatory audits or medical record requests.
Audits medical records each week for compliance with regulatory process and outcome measures for all phases of transplantation and donation.
Completes incident reports for transplant leadership on all discrepancies found.
Maintain confidentiality of patient information.
Communicates with Organ procurement agencies, managed care organizations, physician offices, patients and regulatory agencies.
Conducts data analysis, maintains transplant statistics and prepares data for monthly performance improvement meetings for all programs.

Troubleshoots IT needs for administrative and clinical team members and escalates to IT if unable to resolve Completes work within an acceptable time frame.
Prioritizes/organizes work assignments and adjust priorities based on changing situations Promotes/maintains a safe, clean and orderly environment Prepares and collects data for the transplant quality meetings and completes weekly and monthly chart audits Required EducationBachelor's degree required or may substitute 10 or more years of transplant data management experience with high school/GED.
