Clinical Social Worker
Responsible for facilitating complex discharge planning with patient/family and treatment team. Responsible to provide the patient, family and staff any resources relevant to patient's illness and discharge needs. Demonstrates the skill and knowledge necessary to assure assessment, treatment plan and interventions are age appropriate for the unit served. Expected to maintain behavior, judgment and attitude which reflect professional practice standards. Provides psychosocial assessments, crisis intervention, social work consultation, referrals, short term crisis counseling to facilitate discharge plans, and/or adjustment to illness.
Special Requirements:
Requires a Master Degree in Social Work (MSW). Current NJ SW License - (NEW JERSEY SOCIAL WORKER), LCSW Preferred.
Estimated Salary: $20 to $28 per hour based on qualifications.
