Short DescriptionThe Advanced Practice Provider (APP) provides in-home post hospital follow up care to adult patients recently discharged from the hospital or skilled nursing facility.
An APP home visit affords high risk patients access to proactive, comprehensive, compassionate, and collaborative care to improve their health and well-being, while keeping them home.
Responsibilities include but are not limited to:
Perform in-home comprehensive physical and social patient assessments.
Develop a comprehensive treatment plan for each patient.
Medication Reconciliation and management.
Order and interpret diagnostic tests.
Communicate test results to patients and their caregivers.
Provide patient and care giver education.
Collaborate with the interdisciplinary team.
Partner with population health for continued care coordination.
Document in patient's electronic health record.
Experience RequiredMinimum of 3yrs, working with the adult and geriatric population.
Current knowledge and understanding of Primary Care, Chronic Care Management, and Best Practices.
Home Based Primary Care Experience preferred.
Education RequirementsMaster's Degree in Nursing or Physician Assistant degree is required.
License/Certification RequirementsCertification:
American Heart Association BLS certification is requiredLicense:
NJ-APN & RN or NJ PA License is requiredValid State Driver's License with a clean DMV record.
Copy of recent Vehicle Insurance required.
