Short DescriptionThe Manager Program position within the Urba Health Institute oversees the innovation work within the institute.
This position is responsible for oversight of the care management team providing services to medically and socially complex patients, operations of the Shared Medical Appointments, supervision of student learners, and coordination and collaboration with internal and external partnersDevelop workflows for Health Coach Care Management activities, provide staff training, implement workflows, determine outcomes metrics, and track staff productivity.
Work with Medical Director and Lead APN to identify opportunities for Shared Medical Appointments.
Meet with interested parties, conducted survey for SMA development.
Operationalize SMA:
identify staffing needs, recruitment strategies, scheduling templates and EPIC documentation needs.
Lead team meetings; set tactical direction and tasks; coach and mentor team and manage their performance.
Collaborate with Educational Programs to facilitate and support student learning opportunitiesAssist in obtaining grant opportunities to continue innovation work within the Urban Health Institute and support grant-related initiatives as neededServe on committees within the health system and/or in the communityCollaborate with internal and community partners in the management of mutual patientsPerforms all related duties or special projects as assigned/requiredExperience RequiredExperienceRequiredPreferredSpecialConsiderations ExperienceRequiredPreferredSpecialConsiderations0-2 years6-10 years3-5 years10

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
