How you can make an impact:
As a Licensed Clinical Social Worker you will provide Behavioral Health consultation as part of an interdisciplinary team-based model of care.
Our patients receive care in the OBGYN, Pediatrics, Internal Medicine, MAT, and Dental Departments.
You will be a part of our growing mission-driven organization that thrives on collaboration, novel thinking, and transformation.
What you will do:
Conduct brief/short-term/intermediate therapy for individuals and families Conduct biopsychosocial assessments and be familiar with other clinical assessments and use of the DSM in diagnosing and treatment Establish and formulate comprehensive treatment goals/plans Complete timely and accurate documentation in EMR Actively participate in case conferences with behavioral health, social work staff, and other clinical staff Make appropriate patient referrals for psych evaluations and medication management services Maintain and manage provider schedule and maintain active patient list (caseload) Attend monthly departmental meetings and supervision (if applicable) Educate patients in the health center and community in the nature of behavioral, mental, and social health determinants on mental health Who you are and the expertise you bring:
Compassion for helping patients.
Must possess a current NJ License and or Certification:
LCSW, LPC, LCADC Credentialling with Medicaid, Medicare, and other Insurance panels a plus Must have at least 3-5 years of experience working in a behavioral health setting providing individual and family counseling.
Strong background and experience in working with diverse populations Experience working with children, adolescents, and adults Must maintain continuing education requirements Bilingual and fluent in Spanish is a plus CAMcare's COVID-19 vaccine mandate is in alignment with applicable local, state and federal mandates.
